(a)
A form used to apply for registration at a license branch must comply with
52 U.S.C.
20504
(c)(2)
and
52 U.S.C.
20504
(d)
.
(b)
A form used to apply for registration at:
(1)
a public assistance agency designated under IC
3-7-15
;
(2)
an agency serving persons with disabilities designated under IC
3-7-16
;
(3)
an additional office designated under IC
3-7-18
or IC
3-7-19
; or
(4)
an office of the department of employment and training services designated under IC
3-7-20.5
;
must comply with
52 U.S.C.
20506

.
