Iowa Regulations § 721-21.52 Request for new voter identification card
Regulation Text
(1)
If a voter's identification card is lost or damaged, the registered voter may request a new card in person at the commissioner's office by showing identification, or by a written, signed request to the commissioner's office. Upon receiving the request, the commissioner shall print and mail a new voter identification card.
(2)
If the voter appears in person but does not have the correct form of identification, the commissioner shall verify the voter's identity by asking the voter to provide at least two of the following personal facts:
a.
Date of birth;
b.
Last four digits of the voter's social security number (if the number is stored within I-Voters);
c.
Driver's license or nonoperator's identification card number (if the number is stored within I-Voters);
d.
Address;
e.
Middle name;
f.
Voter verification number pursuant to Iowa Code section
53.2

Upon the successful verification of the voter, the commissioner shall issue a new copy of the voter identification card over the counter. If the voter is unable to respond correctly to at least two of the questions in this subrule, the commissioner shall not issue a copy of the voter identification to the voter.
This rule is intended to implement 2017 Iowa Acts, House File 516, section 18.
