Nevada Regulations § 293.081 Change of manner in which name of candidate appears on ballot

1. For the purposes of NRS 293.2565 , the surname of the candidate printed on a ballot may be his or her original surname or married surname. A candidate may not request that a new modification or combination of his or her original surname and married surname appear on a ballot.
2. A candidate may change the manner in which his or her name will appear on the ballot by filing a written request to amend his or her declaration of candidacy. The request must be filed by the candidate in person with the filing officer with whom he or she filed the declaration of candidacy not later than 5 p.m. on the last day to file the declaration of candidacy.
This subsection does not authorize the amendment of any matter contained in a declaration of candidacy other than the manner in which the name of the candidate appears.
