The candidate's request to withdraw nomination must be in the following form:
CANDIDATE'S REQUEST TO WITHDRAW NOMINATION
I, ________________________________________________________, candidate for the office of ___________________________________________________________________________ (list office, party and/or district number, if applicable), request that my nomination be withdrawn and that my name not appear on the ballot. I, under oath, verify that I have read and understand the contents of this request to withdraw my candidacy.
