The declaration of candidacy and certification of running mate for independent candidates for president and governor must be in the following form:
INDEPENDENT CANDIDATE FOR PRESIDENT OR GOVERNOR
DECLARATION OF CANDIDATE
AND
CERTIFICATION OF RUNNING MATE
I, __________________________ (name of candidate), under oath, declare that I am eligible to seek the office of _____________________ (insert President of the United States or Governor). I certify that I have named ________________________ as my _______________________ (insert Vice President or Lieutenant Governor) candidate.
