The statement for precinct committeeman or committeewoman shall be in the following form:
Precinct Committeeman or Committeewoman Statement

My mailing address is ________________ , __________ , SD ________ my residence address is ___________________ , ___________ , SD ________ , my email address is __________ , and my telephone number is _______________.
I understand that to be a Precinct Committeeman or Committeewoman, I am a resident of the precinct, I am a registered voter of the political party named above at the time I sign this statement, and if elected, will serve in the office.
