When anyone returns a voted ballot in person, the auditor's receipt for that absentee ballot shall be in the following form:
RECEIPT FOR ABSENTEE BALLOT
I acknowledge receipt of the absentee ballot voted by _____________________ whose voting precinct is ____________ and whose permanent residence is _____________________________. I received this absentee ballot from _____________________.
