The certificate of election for a municipal candidate on the ballot shall be in the following form:

CERTIFICATE OF ELECTION
THIS IS TO CERTIFY that on the ________ day of __________, 20____, at the municipal election held throughout the city of ______________
was duly elected by the qualified voters of the city of __________ to the office of ______________ for a term of ______ year(s) beginning ______________.
